Job summary

Interim HealthCare Inc. is seeking a Hospice Client Care Coordinator (CCC) to support a full range of patient services, delivering comfort and dignity to our clients.

The role focuses on enrollment, screening assessments, and maintaining comprehensive healthcare records while coordinating with RNs, pharmacies, and families. Applicants should have a High School diploma, at least 1 year of related business experience, and a valid driver’s license.

Qualifications

• High School diploma or equivalent is required.
• At least 1 year of related business experience or education.
• Experience in senior care or hospice is preferred.
• Must have a valid driver’s license.

Responsibilities

• Facilitate new client enrollment and initial/follow-up screening assessments and maintain client records.
• Identify resources to address clients' health, daily living, and safety needs.
• Perform scheduling, logging, and documentation of all appointments and services.
• Communicate with clients via telephone and computer.
• Coordinate with RN and pharmacy for medication set-ups and reminders.
• Serve as a family liaison and coordinate with family, designees, and providers.

Asa Hospice Client Care Coordinator (CCC), you will:

• Facilitate new client enrollment withinitialandsubsequentscreening assessmentsand furthermaintainclient’s healthcare record

• Perform administrative tasks such as scheduling, logging, documenting all appointments and services
• Communicateto clients through telephone and computer
• Coordinate with RN and pharmacy medication set-ups and reminders
• Be a family liaison and coordinate with the family, designees, and providers

To qualifyto be a Hospice Client Care Coordinator (CCC) with us, you will need:

• Education:High School graduate or equivalent
• One year of related business experience or equivalent of education experience
• Experience in working in the senior field or hospice is preferred
• Must have a valid driver’s license
