Job summary

Interim HealthCare Hospice & Palliative Care is seeking a compassionate Hospice CNA to provide direct patient care per the plan of care, in home and community settings, delivering comfort and dignity to our clients.

Qualifications

• Certified Nurse Assistant Certification required.
• Attention to detail; able to carefully follow instructions and document notes regarding a patient's condition.
• Reliable Transportation with Proof of Valid Driver's License and Auto Insurance

Responsibilities

• Provide direct patient care and assistance in accordance with plan of care.
• Aiding with activities of daily living (i.e., bathing, dressing, oral hygiene, skin care)
• Recognizing and reporting abnormal findings or changes in patient status.
• Working as part of an interdisciplinary team that helps establish and carry out their plan of care to provide comfort and dignity throughout their diagnosis
