Transition Home Healthcare in Spring, TX is seeking a Patient Care Coordinator (PCC) to manage patient admissions and care planning. You will handle intake calls, referrals, medical records, and schedule field staff while collaborating with the clinical team to ensure timely admissions.

The ideal candidate brings 1–2 years of home health coordination, proficiency with Homecare Homebase (HCHB), knowledge of hospice regulations and Medicare guidelines, and excellent communication skills.

• Knowledge of hospice regulations, Medicare guidelines.
• Excellent communication and customer service skills.
• High level of accuracy and attention to detail.
• Able to multitask in a high-volume environment.
• Must be organized, proactive, and solution-focused.

Responsibilities

• Receive and process all patient referrals and intake inquiries.
• Coordinate and verify insurance, physician orders, and required documentation.
• Maintain and manage up-to-date patient medical records and files in compliance with regulatory standards.
• Serve as the primary admissions coordinator, facilitating timely evaluations and initiations of care.
• Schedule field staff visits in alignment with patient needs and staffing availability.
• Collaborate with Administrator and interdisciplinary team to ensure well-coordinated care.
• Serve as volunteer coordinator and liaison between facilities, patients, families, and hospice team.
• Monitor and follow up on pending admissions or incomplete documentation.
• Use Homecare Homebase (HCHB) to track and manage patient information and referrals.

The Job:

The Home Health Patient Care Coordinator plays a key role in coordinating all aspects of the patient admission and care planning process. This includes managing intake calls, processing referrals, overseeing medical records, scheduling field staff, and collaborating with the clinical team to ensure smooth, timely admissions. The ideal candidate must have prior experience using Homecare Homebase and be comfortable working in a growth environment.

Primary Responsibilities:

• Receive and process all patient referrals and intake inquiries
• Coordinate and verify insurance, physician orders, and required documentation
• Maintain and manage up-to-date patient medical records and files in compliance with regulatory standards
• Serve as the primary admissions coordinator, facilitating timely evaluations and initiations of care

• Collaborate closely with the Administrator, and interdisciplinary team to ensure patient care is well-coordinated and efficient
• Serve as Volunteer coordinator and liaison between, facilities, patients, families, and the hospice team
• Monitor and follow up on pending admissions or incomplete documentation
• Utilize Homecare Homebase (HCHB) to track and manage patient information, and use of forcura for monitoring faxes and referrals
• Providing an outstanding admissions experience for our patients
• Communicating effectively with patients, family members, Case Managers, healthcare providers and co-workers
• Reviewing patient charts and ensuring information is clearly communicated to the patients care team
• Serving as a point of contact throughout the admissions process
• Ensuring coordination of care with other team members

Must-have’s to be considered for this position:

• Minimum 1-2 years of home health coordination required
• Proficient inHomecare Homebase (HCHB), Kinserr
• Strong knowledge of hospice regulations, documentation standards, and Medicare guidelines
• Excellent communication and customer service skills
• High level of accuracy and attention to detail
• Able to multitask and prioritize in a high-volume environment
• Must be organized, proactive, and solution-focused
• High School Diploma or equivalent required
