Job summary

Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our team. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities, and in private homes.

We value kindness, respect, compassion, and dignity for residents, families, and staff. Ideal candidates will have an HHA license (NJ), understand the hospice philosophy, have at least one year of hospice experience, and dependable transportation to cover various care

Qualifications

Minimum of one year experience working in hospice.
Understands the hospice philosophy.
Dependable transportation required.

Responsibilities

Assists with prescribed exercise and ROM activities as taught by Hospice Nurse.
Assists with toileting, positioning and transfer of residents/patients.
Provides or assists in ADL activities: personal hygiene, grooming, mouth, skin care and nails.
Assists or reminds patient to take medications in compliance with regulations.
Performs light housekeeping of the resident's/patient's unit, including changing bed linen and patient laundry.
Feeds and assists with meals.
Obtains temperature, pulse and respiration as supervised by the Hospice Nurse.
Reports changes in resident's/patient's condition to Hospice Nurse and documents information with date/time.
