Job summary

Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our care team. We deliver compassionate support to patients in skilled nursing facilities, assisted living facilities, and private homes.

Ideal candidates hold an HHA license in NJ, understand hospice philosophy, have at least one year of hospice experience, and possess dependable transportation.

Qualifications

• Ideal candidates will have an HHA license in the state of NJ.
• Understand hospice philosophy.
• Minimum of one year experience working in hospice.
• Dependable transportation.

Responsibilities

• Assists with prescribed exercise and ROM activities as taught by the Hospice Nurse.
• Provides assistance in toileting, positioning and transfer, turning a bed-bound resident.
• Assists with ADL activities such as personal hygiene, grooming, mouth, skin care and nails.
• Assists or reminds patient to take medication in compliance with regulations.
• Assists with light housekeeping and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respiration as supervised by the Hospice Nurse.
• Reports changes in resident's condition to the Hospice Nurse and documents information.
