Job summary

Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our caring team. We serve residents in skilled nursing facilities, assisted living facilities, and private homes with compassion and dignity.

Ideal candidates hold an NJ HHA license, understand hospice philosophy, have at least one year of hospice experience, and possess dependable transportation to reach patients in various settings.

Qualifications

• HHA license in NJ is required.
• Hospice experience is preferred.
• Minimum of one year experience in hospice.
• Dependable transportation or reliable means to travel to patient locations.

Responsibilities

• Assist with prescribed exercises and ROM as taught by Hospice Nurse.
• Provide assistance in toileting, positioning, transfers, and bed mobility.
• Assist with ADLs: hygiene, grooming, mouth care, skin care, nails.
• Assist with medication reminders in compliance with regulations.
• Assist with light housekeeping of the patient’s unit or room and laundry.
• Feed and assist with meals.
• Record vitals (temperature, pulse, respiration) as instructed by the Hospice Nurse.
• Report any changes in patient condition to the Hospice Nurse and facility nurse as applicable.
