Qualifications

• Hospice experience is required.
• Experience as a Home Health Aide is preferred.

Responsibilities

• Assists with prescribed exercise and ROM activities as taught by the Hospice Nurse.
• Provides assistance in toileting, positioning and transfer actions for a bed bound resident/patient.
• Performs ADL activities including personal hygiene, grooming, skin care and nails.
• Assists or reminds patient to take medications in line with regulations.
• Helps with light housekeeping and patient laundry as needed.
• Feeds and assists with meals.
• Obtains temperature, pulse and respiration under supervision.
• Reports changes to the Hospice Nurse and documents details as required.
