Swan Hospice is seeking a Certified Home Health Aide (CHHA) with hospice experience to join our team. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. Qualifications

• HHA license in NJ.
• Minimum of one year experience in hospice.
• Understands the hospice philosophy.
• Dependable transportation.

Responsibilities

• Assists with prescribed exercise and ROM activities under supervision.
• Provides assistance in toileting, positioning and transfers.
• Performs ADL tasks such as personal hygiene, grooming, skin care and nails.
• Reminds or assists patient with medications per regulations.
• Light housekeeping of patient unit, including changing bed linen and laundry.
• Feeds and assists with meals.
• Measures temperature, pulse and respiration as directed.
• Reports changes to Hospice Nurse and documents the information.
