Job summary

VitalCaring is seeking a Hospice Chaplain / Spiritual Counselor to provide compassionate, individualized spiritual care and emotional support to patients and families during end-of-life care. You will align care with beliefs and preferences, while supporting families through grief and transitions.

Field-based visits in homes and facilities with strong interdisciplinary collaboration, documentation, and ongoing professional development in a purpose-driven hospice environment.

Qualifications

• Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree) required.
• Knowledge of hospice philosophy of care.
• Ability to work effectively within an interdisciplinary team.
• Comfort serving within an interfaith setting and diverse cultures.
• Excellent communication, empathy, and active listening.
• Experience with death, dying, grief, and family support.
• Valid driver’s license and reliable transportation for field visits.

Responsibilities

• Assess spiritual needs and provide individualized care plans.
• Offer spiritual counseling, emotional support, and grief support.
• Support patients and families through difficult conversations and transitions.
• Collaborate in IDT meetings and care planning with the care team.
• Maintain accurate clinical documentation and quality initiatives.
