Job summary

Crossroads Hospice & Palliative Care in Cleveland, OH seeks a dedicated Hospice Chaplain to provide spiritual care to patients and families at the end of life. You will assess spiritual needs, coordinate support, and collaborate with the interdisciplinary team to honor diverse faith traditions.

This role emphasizes compassionate listening, bereavement follow-up, and participation in care conferences. Prior CPE and ecclesiastical endorsement are required or preferred as outlined.

Qualifications

• Ecclesiastical endorsement by faith group/denomination.
• Minimum two units of Clinical Pastoral Education (CPE) from an ACPE-accredited center.
• Master of Divinity degree or equivalency preferred.
• Nonjudgmental and inclusive presence.
• Spiritual care experience with terminal patients, families, and clinicians.
• Knowledge of hospice philosophy of care.
• Awareness of diverse end-of-life faith practices.
• Hospice care experience preferred (one-year minimum).
• Excellent verbal and written communication skills.
• Ability to work independently and on hospice care teams.
• Ability to use electronic medical resources for care plans and documentation.
• Meet federal and state background check requirements.
• Proof of valid driver’s license and reliable transportation.

Responsibilities

• Provide or coordinate spiritual care support to patients, family members, and caregivers.
• Conduct spiritual assessments of patients and families in a timely manner as required.
• Assist with bereavement follow-up care in collaboration with the bereavement coordinator.
• Participate in care conferences, hospice admissions, and IDG meetings.
• Provide emotional/spiritual support to hospice team members as needed.
• Develop and maintain relationships with local spiritual leaders.
• Participate in quality improvement programs and services.
• Participate in weekend on-call rotation as designated.
