Qualifications

  • Ecclesiastical endorsement by faith group/denomination.
  • Minimum two units of Clinical Pastoral Education (CPE) from an ACPE-accredited center.
  • Master of Divinity degree or equivalency preferred.
  • Nonjudgmental and inclusive presence.
  • Spiritual care experience with terminal patients, families, and clinicians.
  • Knowledge of hospice philosophy of care.
  • Awareness of diverse end-of-life faith practices.
  • Hospice care experience preferred (one-year minimum).
  • Excellent verbal and written communication skills.
  • Ability to work independently and on hospice care teams.
  • Ability to use electronic medical resources for care plans and documentation.
  • Meet federal and state background check requirements.
  • Proof of valid driver’s license and reliable transportation.

Responsibilities

  • Provide or coordinate spiritual care support to patients, family members, and caregivers.
  • Conduct spiritual assessments of patients and families in a timely manner as required.
  • Assist with bereavement follow-up care in collaboration with the bereavement coordinator.
  • Participate in care conferences, hospice admissions, and IDG meetings.
  • Provide emotional/spiritual support to hospice team members as needed.
  • Develop and maintain relationships with local spiritual leaders.
  • Participate in quality improvement programs and services.
  • Participate in weekend on-call rotation as designated.

Hospice Chaplain Requirements:
  • Ecclesiastical endorsement by faith group/denomination

  • Proof of valid driver’s license and reliable, insured transportation
Hospice Chaplain Responsibilities:
  • Provide or coordinate spiritual care support to patients, family members, and caregivers
  • Conduct spiritual assessments of patients and family members in a timely manner as required
  • Assist with bereavement follow-up care as appropriate in collaboration with bereavement coordinator
  • Participate as a member of the hospice team in regularly scheduled care conferences, hospice admissions, and Interdisciplinary Group (IDG) meetings
  • Provide emotional/spiritual support to hospice team members as needed
  • Develop and maintain relationships with local spiritual leaders
  • Participate in quality improvement programs and services
  • Participate in weekend on-call rotation as designated
