Job summary

VitalCaring, a home health and hospice provider, seeks a Chaplain / Spiritual Counselor to support hospice families with grief counseling, spiritual needs, and referrals. You will coordinate with local clergy and interdisciplinary teams, documenting services in the EMR.

Ideal candidate holds a BS/BA in theology or related field and a master’s in counseling or theology, with at least three years in spiritual care ministry and hospice experience.

Qualifications

• BS/BA in theology, religion, human services, counseling, psychology, or sociology.
• Master's degree in counseling, psychology, theology or divinity.
• Minimum of three years active work in spiritual care ministry.
• One year of hospice spiritual care and clinical pastoral education preferred.

Responsibilities

• Support hospice families with grief counseling and referrals.
• Facilitate visits by local clergy and pastoral counselors for patient spiritual needs.
• Document services in the electronic medical record (EMR).
• Attend Interdisciplinary Group meetings to coordinate counseling care.
