Qualifications

• Bachelor's degree in religious studies or related subject and a Master's in theology, divinity, or pastoral studies.
• Certification is preferred; ordained or commissioned chaplain with endorsement letter.
• Ongoing commitment toward development through education and obtaining CPE.
• Experience in hospice care preferred and comfort in an interfaith setting.
• Knowledge of hospice philosophy of care; ability to network with community clergy and congregations; CPR certification and valid driver’s license.

Responsibilities

• Assures spiritual assessment of patients and families/caregivers and ensures timely services.
• Participates in comprehensive assessment to develop the individualized plan of care for spiritual needs.
• Provides direct spiritual care to patients and families/caregivers.
• Acts as liaison to community chaplains and spiritual counselors.
• Maintains records of spiritual care services for QA and policy review.
• Documents services and ongoing communication with community chaplains and counselors.
• Attends patient care conferences and interdisciplinary group meetings.
• Provides consultation and support to the interdisciplinary team on spiritual care.
• Recruits community chaplains and spiritual care volunteers to meet patient needs.
• Provides for funeral or memorial services as requested and plans periodic memorial services.
• Supervises spiritual care volunteers as assigned.
• Assists in continuity of care with patients and families.
