Job summary

St. Croix Hospice in Maple Grove, MN seeks a Chaplain to provide emotional, spiritual and practical support to patients and families, coordinating with the care team to ensure dignified end-of-life care.

Qualifications

• Bachelor's degree or 5+ years in a formal ministerial setting is required.
• Theological coursework is required.
• Must pass background, consumer report and drug tests.

Responsibilities

• Conduct spiritual assessments and develop individualized care plans.
• Provide direct spiritual support to patients and families.
• Coordinate with community chaplains and spiritual counselors.
• Maintains records for quality assurance and HIPAA compliance.
• Attends interdisciplinary team meetings and provides spiritual care consultation.
• Recruit and educate community chaplains, counselors, and volunteers.
• Oversee bereavement program and memorial services.
