Job summary

St. Croix Hospice in Minnesota seeks a Chaplain to provide emotional, practical and spiritual guidance to patients and families on their hospice journey, supporting the care team and coordinating with community chaplains.

Qualifications

• Bachelor's degree required; in lieu of a degree, a minimum of 5 years of ministerial experience is required.
• Demonstrated theological coursework required.
• Ability to pass background, consumer report and drug test.
• Valid State Driver’s License, automobile in good working order, and appropriate insurance and driving record.

Responsibilities

• Conduct spiritual assessments and develop individualized care plans.
• Provide direct spiritual support to patients and families.
• Coordinate with community chaplains and spiritual counselors.
• Maintain records for quality assurance and HIPAA compliance.
• Attend interdisciplinary team meetings and provide spiritual care consultation.
• Recruit and educate community chaplains, counselors, and volunteers.
• Assist in developing and maintaining a bereavement program with follow-up care.
• Oversee condolence calls and memorial service scheduling.
• Conduct funeral/memorial services as requested.
• Organize periodic memorial services for staff, volunteers, and clergy.
• Supervise spiritual care volunteers as needed.
• Participate in quality assessment and performance improvement activities.
