Job summary

St. Croix Hospice seeks a Chaplain to provide emotional, practical and spiritual guidance to patients and families, meeting them where they are on their hospice journey.

You will be part of the patient care team, coordinating with clinicians and connecting families to community resources. Responsibilities include conducting spiritual assessments, developing care plans, supervising volunteers, and supporting bereavement services.

Qualifications

• Bachelor's degree required; in lieu of a degree, a minimum of 5 years of experience in a formal ministerial setting is required.
• Demonstrated theological coursework required.
• One or more unit(s) of Clinical Pastoral Education preferred.

Responsibilities

• Conduct spiritual assessments and develop individualized care plans.
• Provides direct spiritual support to patients and families.
• Coordinates with community chaplains and spiritual counselors.
• Maintains records for quality assurance and compliance.
• Attends interdisciplinary team meetings and provides spiritual care consultation.
