Job summary

VitalCaring is seeking a Hospice Chaplain / Spiritual Counselor to provide compassionate spiritual care and emotional support to patients and families during end-of-life journeys. You will tailor spiritual care plans to individual beliefs and values, and collaborate with nurses, social workers, physicians, aides, and clergy to ensure patient-centered care.

This role emphasizes autonomy within a supportive interdisciplinary team, field-based visits, and ongoing professional development in hospice

Qualifications

• Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent degree)
• Strong empathy, listening, and communication skills for end-of-life care
• Ability to work within an interdisciplinary team and with diverse beliefs

Responsibilities

• Provide spiritual counseling and emotional support to patients and families
• Develop individualized spiritual care plans in collaboration with IDT
• Support families through grief, transitions, and end-of-life conversations
• Document care activities accurately in EMR and participate in quality initiatives
