Job summary

The Chapters Health System seeks a Chaplain to provide a broad range of spiritual services to patients and families enrolled in its programs. The Chaplain also provides spiritual support to staff and volunteers.

Develops a spiritual care plan with the interdisciplinary group, delivers care, and coordinates with clergy or counselors as needed, while maintaining documentation and confidentiality in EMR/EHR systems.

Qualifications

• Bachelor's degree in Divinity or related field; Master’s degree preferred.
• Ordination or commissioning/endorsement within 12 months of hire.
• Documentation demonstrating good standing with faith/professional group with no ethical charges.
• One unit of Clinical Pastoral Education (CPE) or in process within six months.
• Experience with EMR/EHR systems preferred; computer skills required.

Responsibilities

• Develops a spiritual care plan with the interdisciplinary team based on patient/family traditions and assessments.
• Provides spiritual care and initiates interventions to resolve care plan issues.
• Engages with Spiritual Care volunteers, local clergy, or counselors as needed to implement the plan.
• Monitors and reassesses care plan; maintains timely, accurate documentation.
• Acts as liaison between the Company and local clergy; participates in team meetings.
• Performs other duties as assigned.
