Job summary

Andwell Health Partners in Norway, ME is seeking a Hospice Chaplain to provide compassionate spiritual support to patients and families navigating end-of-life journeys in homes and facilities.

You will assess bereavement needs, develop individualized care plans aligned with beliefs, document in the EMR, and collaborate with an interdisciplinary hospice team to ensure holistic care. A valid driver’s license and at least one year of chaplain experience are required; CPE preferred.

Qualifications

• Minimum of one year of experience as a Chaplain, preferably in a health care/Hospice setting.
• One unit of Clinical Pastoral Education (CPE) or equivalent preferred.
• Valid Driver's License and reliable car with proof of insurance.

Responsibilities

• Provide compassionate spiritual and emotional support to patients and families as they navigate the end-of-life journey.
• Assess and address bereavement needs of patients, families, and caregivers with sensitivity and care.
• Develop, implement, and update individualized, goal-oriented spiritual plans of care reflecting each patient’s beliefs, values, and preferences.
• Employ a range of counseling and spiritual care techniques to help patients and families find comfort, meaning, and peace.
• Accurately document all assessments, interventions, and communications in the EMR per agency policies and professional standards.
• Collaborate with the interdisciplinary hospice team to ensure coordinated, holistic care that supports the physical, emotional, and spiritual well-being of each patient and family.
