Job summary

Andwell Health Partners in Norway, ME is seeking a compassionate Hospice LPN to join our team and provide patient-centered nursing in the community. You will monitor patient conditions, administer medications, and support families through a challenging time with dignity and respect.

The role requires an active Maine LPN license, strong clinical skills, and the ability to work independently while collaborating with a multidisciplinary team.

Responsibilities

• Provide skilled nursing care to hospice patients in their homes and other care settings.
• Monitor patient conditions and communicate changes to the hospice care team.

• Educate and support patients and families with compassion, patience, and respect.
• Collaborate with RNs, providers, social workers, chaplains, aides, and other interdisciplinary team members.
• Help patients remain comfortable and supported while honoring their goals and preferences.
• Complete accurate clinical documentation and follow hospice standards of care.
