Job summary

Andwell Health Partners is seeking a part-time Hospice Chaplain to provide compassionate spiritual and emotional support for patients and families navigating end-of-life care in home and facility settings. You will assess bereavement needs, develop individualized spiritual plans, and collaborate with the hospice team to ensure holistic care.

A CPE unit is preferred, with 1 year of hospice chaplain experience and a valid driver's license.

Qualifications

• One unit of Clinical Pastoral Education (CPE) or equivalent preferred.
• Minimum of one year of experience as a Chaplain, preferably in health care/hospice settings.
• A valid Driver's License and reliable car with proof of insurance.

Responsibilities

• Provide spiritual and emotional support to patients and families as they navigate the end-of-life journey.
• Assess and address bereavement needs of patients, families, and caregivers with sensitivity.
• Develop, implement, and regularly update individualized, goal-oriented spiritual plans reflecting each patient's beliefs and values.
• Employ counseling and spiritual care techniques to help patients and families find comfort and meaning.
• Document all assessments, interventions, and communications in the EMR per agency policies and professional standards.
• Collaborate with the interdisciplinary hospice team to ensure holistic care for each patient and family.

What You'll Do

• Provide compassionate spiritual and emotional support to patients and families as they navigate the end-of-life journey.
• Assess and address the bereavement needs of patients, families, and caregivers with sensitivity and care.
• Develop, implement, and regularly update individualized, goal-oriented spiritual plans of care that reflect each patient’s unique beliefs, values, and preferences.
• Employ a range of counseling and spiritual care techniques to help patients and families find comfort, meaning, and peace.
• Accurately document all assessments, interventions, and communications in the electronic medical record in accordance with agency policies and professional standards
• Collaborate closely with the interdisciplinary hospice team to ensure coordinated, holistic care that supports the physical, emotional, and spiritual well-being of each patient and family.

What You’ll Bring:
