Job summary

Andwell Health Partners is seeking a Hospice Registered Nurse (RN) to provide skilled nursing care in patients' homes across Farmington, ME and surrounding towns. The role offers per diem scheduling with autonomy and a patient-centered focus.

You will assess needs, create care plans, manage pain and symptoms, educate families, and collaborate with a hospice team to ensure comfort and dignity at every stage of the journey.

Qualifications

• Current RN license in the State of Maine.
• Minimum of one year of recent clinical nursing experience.
• Knowledge of pain management and symptom control practices.
• Strong assessment, communication, and critical thinking skills.
• Compassion, empathy, and a commitment to patient-centered care.
• Reliable transportation and current automobile insurance.

Responsibilities

• Delivering skilled nursing services in patients' homes per physician-directed plans of care.
• Assessing patients' physical, emotional, psychosocial, and spiritual needs.
• Developing individualized care plans with measurable outcomes in partnership with patients and families.
• Managing pain and symptoms to maximize comfort and quality of life.
• Educating patients, caregivers, and family members to support informed decision-making.
