Job summary

Andwell Health Partners is seeking a Hospice Chaplain to provide compassionate spiritual and emotional support to patients and families in home and care facility settings. You will develop individualized spiritual plans of care aligned with each patient’s beliefs and values, and collaborate closely with the interdisciplinary hospice team to ensure holistic care.

The role emphasizes 1:1 patient support, flexibility in scheduling, and autonomy in the home setting, with a comprehensive benefits

Qualifications

• Provide spiritual and emotional support to patients and families facing end-of-life.
• Minimum of one year of chaplain experience, preferably in health care/hospice.
• Valid driver’s license and reliable car with proof of insurance.

Responsibilities

• Provide compassionate spiritual and emotional support to patients and families as they navigate the end-of-life journey.
• Assess and address bereavement needs of patients, families, and caregivers with sensitivity.
• Develop, implement, and update individualized, goal-oriented spiritual plans of care reflecting patients' beliefs and values.
• Utilize counseling and spiritual care techniques to help patients and families find comfort and peace.
• Document assessments, interventions, and communications in the EMR per agency policy.
• Collaborate with the interdisciplinary hospice team to ensure coordinated, holistic care.
