Job summary

Three Oaks Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our team in providing in-home hospice care. You will assist patients in their home with personal care and support families, under the supervision of a registered nurse.

This role emphasizes compassionate listening, patient-centered care, and teamwork across our network. A CNA license and driver’s license are required, with prior hospice or long-term care experience preferred.

Qualifications

• High School Diploma preferred and CNA license required.
• State-approved Nurse Aide training and competent registry standing.
• Must have CNA license and current driver’s license.
• Minimum one year hospice or nursing assistant experience preferred.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Provide personal healthcare and related services at the patient's home.
• Maintain a safe, clean environment and assist family.
• Collaborate with patient, family, and supervisor; report observations.

We are looking for a passionateCertified Nursing Assistant CNAto join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY:Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

QUALIFICATIONS:

• High School Diploma Preferred
• Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements
• Must have CNA license
• Must have current driver’s
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Reliable transportation. Valid and current auto liability insurance.
