Job summary

Three Oaks Hospice is seeking a compassionate Certified Nursing Assistant to join our team providing in-home care for patients. You will assist with daily living activities, monitor patient conditions, and communicate observations to the supervising RN.

Responsibilities include maintaining a safe and clean environment, supporting families, and delivering respectful, patient-centered care in a hospice setting. PRN availability may be required.

Qualifications

• CNA license current and in good standing.
• Active state nurse aide certification or equivalent.
• Must possess a valid driver's license.
• At least one year of hospice, home health, or long-term care experience preferred.

Responsibilities

• Provide personal healthcare and related services in the patient’s home.
• Maintain a safe, clean environment and assist with basic caregiving tasks.
• Collaborate with patient, family, and supervisor to share observations.
• Follow directions from a registered nurse and document care appropriately.

We are looking for a passionate Certified Nursing Assistant CNA to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY: Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment, work cooperatively with patient and family and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse.

QUALIFICATIONS:

• High School Diploma Preferred
• Has successfully completed Nurse aide training/competency program approved by the state and listed in good standing on the state nurse aide registry; or State Licensure program that meets requirements
• Must have CNA license
• Must have current driver’s
• At least one year experience, preferred, as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice, or long-term care facility.
• Reliable transportation. Valid and current auto liability insurance.
