Job summary

Heart to Heart Hospice is seeking a dedicated Hospice Aide (CNA) to provide direct patient care under RN supervision. You will assist with transfers, monitor patient condition, and document care to maintain continuity of services.

A caring heart and strong communication with patients and families are essential. Requirements include an unencumbered CNA license, high school diploma or GED, and at least one year of hospice or nursing assistant experience.

Qualifications

• Must have an unencumbered CNA license per applicable registry.
• High school diploma or GED preferred.
• At least one year of hospice/acute care experience.
• Must have current driver's license and reliable transportation.

Responsibilities

• Provide direct patient care under RN direction and care plan.
• Assist with safe transfers and ambulation according to policy.
• Document changes and care provided for continuity of care.
• Practice infection control principles.
• Communicate with patient family and staff respectfully.
• Submit documentation timely.
• Perform other duties as required.
• Ensure compliance with laws and policies.

Hospice Aide - CNA

What You Must Have:

• High School Diploma preferred or GED
• Must have an unencumbered certification according to the applicable Nurse Aide Registry
• Licensure: Current CNA license
• At least one year experience as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long term care facility
• Demonstrates interest in the welfare of ill and elderly; a heart for providing compassionate care
• High integrity, including maintenance of confidential information
• EMR experience a plus
• Must have current driver's license, reliable transportation, and provide ongoing valid and current auto liability insurance

What You Will Do:

• Provide direct patient care to patient under direction of the RN and according to the Hospice Aide/Homemaker Plan of Care.
• Provide necessary skills to assist the patient with safe transfers and ambulation per agency policy
• Provide necessary skills to appropriately report/communicate changes and document pertinent information and care provided to patient to ensure continuity of care.
• Practice accepted infection control principles.
• Promote positive, supportive, respectful communication to patient/family and other employees
• Timely submission of all documentation
• Performance of other duties as required
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures
