Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our growing team, providing hands-on personal care and comfort to hospice patients in homes and care facilities within Champaign and Danville areas. You will work with an interdisciplinary team to support dignity, comfort, and quality of life.

Qualifications

• Active Certified Nursing Assistant (CNA) certification.
• Previous CNA experience required; hospice, home health, long-term care, or skilled nursing experience preferred.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).
