Job summary

The Care Team Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our team, providing hands-on personal care and comfort to hospice patients in their homes and care facilities across Lake, Porter, Jasper, and LaPorte counties.

You will work with an experienced interdisciplinary team to maintain patient dignity and quality of life while supporting families through the hospice journey.

Qualifications

• Active Certified Nursing Assistant (CNA) certification.
• Previous CNA experience required; hospice/home health/long-term care or skilled nursing experience preferred.
• Valid driver's license, reliable transportation, and current automobile insurance.

Responsibilities

• Provide personal care, bathing, grooming, hygiene assistance, and comfort measures in accordance with each patient's individualized plan of care.
• Observe, document, and promptly report changes in patient condition to the supervising nurse.
• Support patients and families with compassion, dignity, and respect while promoting comfort and quality of life.
• Assist with mobility, positioning, and activities of daily living while maintaining a safe care environment.
• Complete accurate and timely documentation of all patient care provided within the electronic medical record (Homecare Homebase).

Schedule, Hours & Territory

• No Nights
• No Weekends
• No On-Call
• Serving patients within approximately a one-hour of Branch
