Job summary

The Care Team Hospice in Mishawaka, IN is seeking a compassionate CNA to provide personal care and comfort-focused support to hospice patients in homes and facility settings, working with an interdisciplinary team to ensure dignity and quality of life.

Schedule is Monday–Friday, 8:00 AM to 4:30 PM with up to a 1-hour radius around Mishawaka. Benefits include tuition reimbursement, 401(k) with employer match, and HSA eligibility.

Qualifications

• Active CNA certification is required.
• Hospice experience preferred.
• Experience using HCHB (Homecare Homebase) preferred.

Responsibilities

• Document care provided accurately and timely in the EMR HCHB.

The Care Team Hospice is seeking a CNA to provide personal care and comfort-focused support to hospice patients in their homes and facility settings while working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Up to a 1-hour radius around Mishawaka, IN
