Job summary

The Care Team Hospice is seeking a Certified Nursing Assistant (CNA) to provide personal care and comfort-focused support to patients in home and facility settings, working with our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM, with an up to 1-hour radius around Mishawaka, IN 46545.

Qualifications

• Active CNA certification required.
• Hospice aide experience preferred.
• Experience using HCHB (Homecare Homebase) preferred.

Responsibilities

• Provide personal care, hygiene, and comfort measures.
• Observe and report changes in patient condition.
• Support patients and families with compassion and respect.
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase).

The Care Team Hospice is seeking a CNA to provide personal care and comfort-focused support to hospice patients in their homes and facility settings while working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Up to a 1-hour radius around Mishawaka, IN

Field Role Requirements

• Active driver’s license
• Ability to pass a criminal background check
