Job summary

The Care Team Hospice is seeking a Certified Nursing Assistant to provide personal care and comfort-focused support to hospice patients in homes and facility settings while collaborating with our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM, with coverage in Bethlehem, PA to Allentown, PA within the Lehigh Valley area.

Qualifications

• Active CNA certification
• Hospice aide experience preferred
• Experience using HCHB (Homecare Homebase) preferred

Responsibilities

• Provide personal care, hygiene, and comfort measures
• Observe and report changes in patient condition
• Support patients and families with compassion and respect
• Document care provided accurately and timely in the EMR HCHB (Homecare Homebase)

The Care Team Hospice is seeking a CNA to provide personal care and comfort-focused support to hospice patients in their homes and facility settings while working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

Schedule, Hours & Territory

• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
• Bethlehem, PA to Allentown, PA - Lehigh Valley area
