Job summary

The Care Team Hospice seeks a dedicated Certified Nursing Assistant (CNA) to provide personal care and comfort-focused support to hospice patients in homes and facility settings, collaborating with our interdisciplinary team to uphold dignity and quality of life throughout the hospice journey.

Standard hours are Monday through Friday, 8:00 AM to 4:30 PM, serving Bethlehem and Allentown in the Lehigh Valley.

Qualifications

• Active CNA certification required.
• Hospice aide experience preferred.
• Experience using HCHB (Homecare Homebase) preferred.
• Active driver’s license required.
• Ability to pass a criminal background check and MVR.

Responsibilities

• Provide personal care, hygiene, and comfort measures.
• Observe and report changes in patient condition.
• Support patients and families with compassion and respect.
• Document care provided accurately and timely in the EMR HCHB.
