Job summary

The Care Team Hospice is seeking a Certified Nursing Assistant (CNA) to provide personal care and comfort-focused support to hospice patients in their homes and facility settings, working alongside our interdisciplinary team to ensure dignity, compassion, and quality of life throughout the hospice journey.

Standard schedule is Monday through Friday, 8:00 AM to 4:30 PM, in the Lehigh Valley area (Bethlehem to Allentown).

Qualifications

• Active CNA certification is required.
• Hospice aide experience is preferred.
• Experience using HCHB (Homecare Homebase) is preferred.

Responsibilities

• Provide personal care, hygiene, and comfort measures.
• Observe and report changes in patient condition.
• Support patients and families with compassion and respect.
• Document care provided accurately in the EMR HCHB.

Field Role Requirements

• Active driver’s license
• Ability to pass a criminal background check
• Ability to pass a Motor Vehicle Record (MVR) check
• Current TB testing in accordance with agency policy
• Current CPR certification as required for the role
