Holy Cross Home Care and Hospice in Maryland is seeking a Hospice Certified Nursing Assistant (CNA) to provide paraprofessional home care services under the direction of the nurse and physician. • Maryland CNA certification required

• Provide paraprofessional services to home care clients as directed by the primary nurse and physician.
• Deliver meaningful 1:1 patient care and build real relationships.
• Document care efficiently using Epic EHR system to coordinate care.

As a Hospice CNA you will provide paraprofessional services to the organization’s home care clients as developed by the primary nurse and as prescribed by the physician and in compliance with the state’s Nurse Practice Act, and any other regulations defining home health aide / hospice aide services.

Minimum Qualifications

• Certified Nursing Assistant (CNA) certification in the State of Maryland

• High school diploma or GED.

• Previous experience in hospice, palliative care, or home health (preferred).

• Compassionate, patient-focused, and able to provide emotional support to patients and families.

• Reliable transportation and a valid driver’s license.
