Job summary

Ascend Hospice & Palliative Care PA is seeking a Certified Nursing Assistant (CNA) in Montgomery County, PA for a day-shift role (M-F 8:30AM-5PM). You will provide resident care, ADL support, toileting, transfers and basic health monitoring as part of a compassionate hospice team.

Must hold a State Certified CNA license; hospice or home health experience is preferred though not mandatory. Drivers license and reliable transportation are required.

Qualifications

• State Certified CNA license required.
• Preferred hospice or home health experience.
• 1 year experience preferred but not required.

Responsibilities

• Assist residents with ADL and personal care, including grooming, hygiene, and nails.
• Support toileting, transfers, and bed positioning as directed.
• Assist with exercises and ROM per Hospice care plan.
• Help with light housekeeping and personal laundry as needed.
• Assist with meals and feeding.
• Record vitals under supervision and report changes to the Hospice Nurse.
• Remind and assist patients with medications per regulations.

Certified Nursing Assistant (CNA) - Montgomery County, PA

Hours: M-F 8:30AM-5PM

This Certified Nursing Assistant will be responsible for, but not limited to:

• Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails.
• Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient.
• Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse.
• Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry.
• Feeds and assists with meals.
• Obtains temperature, pulse and respirations as supervised by the Hospice Nurse.
• Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information on the Home Health Aide Certification of House form, including the date and time of the report and the person receiving the information.
• Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies.

Education

High School Diploma or equivalent

Professional Experience

• Must have a State Certified CNA license
• Experience in hospice or home health, preferred
• No experience necessary but 1 year, preferred
• Must be willing to travel
• Must have a valid driver's license and reliable transportation
