Job summary

Buckeye Hospice is seeking a registered nurse to serve as Case Manager in Youngstown and surrounding locations. You will assess patients in nursing facilities and home settings, develop and implement the plan of care, and coordinate with a multidisciplinary team.

The role requires an RN license, 1–4 years hospice or home health experience, a valid driver’s license, and on-call rotation.

Qualifications

• RN license is required.
• Valid driver’s license and reliable insured transportation.
• 1–4 years RN hospice or home health experience.
• On-call rotation.

Responsibilities

• Provides initial and comprehensive nursing assessment of patient needs.
• Implements and reevaluates the plan of care to meet patient nursing needs.
• Communicates patient progress with patient/caregiver and coordinates with care team.
• Assessments and care of hospice patients in nursing facilities and home settings.
