Job summary

Buckeye Hospice and Palliative Care Services is seeking an experienced Case Manager (RN) to coordinate and deliver nursing care for patients in nursing facilities and private homes in Youngstown and surrounding locations.

The RN will perform initial and ongoing assessments, implement and revise care plans, and collaborate with patients, families and other professionals to ensure compassionate, high-quality hospice care. On-call rotation is required.

Qualifications

• RN License is REQUIRED.
• Valid Driver's License and reliable transportation.
• Minimum of 1 to 4 years as an RN working with hospice services or home health agency.
• On-Call Rotation.
