Qualifications

• RN License is REQUIRED.
• Valid Driver's License and reliable transportation.
• Minimum of 1-4 years of experience as a Registered Nurse in hospice/home health.
• On-Call Rotation.

Responsibilities

• Provide initial and comprehensive assessments of patient needs, developing personalized care plans.
• Implement and revise care plans as needed to ensure quality care.
• Communicate with patients, families, and healthcare teams to coordinate care delivery.
• Supervise and instruct direct care staff and hospice aides to deliver quality care.
• Participate in on-call rotation, providing 24/7 coverage for patients and families.
