Qualifications

• Valid RN license is REQUIRED.
• Must have a valid driver's license and insured transportation.
• Minimum 1–4 years of RN experience in hospice or home health.

Responsibilities

• Perform initial and comprehensive patient assessments and develop care plans.
• Implement and revise care plans to ensure high-quality care.
• Communicate with patients, families, and health teams to coordinate care.
• Supervise direct care staff and hospice aides to deliver quality care.
• Participate in on-call rotation to provide 24/7 coverage.
