Job summary

Buckeye Hospice is seeking a registered nurse to serve as Case Manager in Youngstown and surrounding locations. You will assess patients in nursing facilities and home settings, develop and implement the plan of care, and coordinate with a multidisciplinary team.

The role requires an RN license, 1–4 years hospice or home health experience, a valid driver’s license, and on-call rotation.

Job description

As a Case Manager (RN) for Buckeye Hospice at our Youngstown and surrounding locations you will be responsible for the assessment and care of Hospice patients in nursing facilities and home settings.

Job Responsibilities

• Provides the initial assessment and comprehensive nursing assessment of patient needs.
• Implements the plan of care and reevaluates and revises the plan of care relative to the patient's nursing needs to provide appropriate, comprehensive and responsive goals and interventions.
• Communicates the patient's progress with the patient/caregiver and other disciplines as well as directs, supervises and instructs direct care staff members and nonprofessional hospice aide staff to deliver quality care and coordinates delivery of care services.
• Assessment and care of Hospice patients in nursing facilities and home settings.

Job Requirements

• RN License is REQUIRED.
• Valid Drivers License and reliable transportation that is insured in accordance with Ohio state requirements.
• Minimum of 1 to 4 years as a RN working with hospice services or home health agency.
• On-Call Rotation.
