Job summary

Buckeye Hospice and Palliative Care Services is seeking a dedicated Case Manager (RN) to oversee patient care in Youngstown and surrounding locations. You will perform initial and ongoing nursing assessments, develop and adjust the plan of care, and coordinate services with patients, families, and the care team.

Ideal candidates have an active RN license, 1–4 years of hospice or home health experience, and are prepared for on-call rotations.

Qualifications

• RN License is required.
• Valid Driver's License and insured vehicle.
• 1–4 years RN hospice/home health experience.
• On-Call rotation.

Responsibilities

• Provides the initial assessment and comprehensive nursing assessment of patient needs.
• Implements the plan of care and reevaluates and revises the plan of care relative to the patient’s nursing needs to provide appropriate, comprehensive and responsive goals and interventions.
• Communicates the patient’s progress with the patient/caregiver and other disciplines as well as directs, supervises and instructs direct care staff members and nonprofessional hospice aide staff to deliver quality care and coordinates delivery of care services.
• Assessment and care of Hospice patients in nursing facilities and home settings.

As a Case Manager (RN) for Buckeye Hospice at our Youngstown and surrounding locations you will be responsible for the assessment and care of Hospice patients in nursing facilities and home settings.

Job Requirements

• RN License is REQUIRED.
• Valid Drivers License and reliable transportation that is insured in accordance with Ohio state requirements.
• Minimum of 1 to 4 years as a RN working with hospice services or home health agency.
• On-Call Rotation.
