Job summary

Buckeye Hospice and Palliative Care Services in Athens, Ohio seeks a compassionate RN Case Manager to deliver patient-centered care, assess needs, and coordinate services.

You will participate in on-call rotations and support families while promoting independence and dignity. This role covers multiple locations including Zanesville area with weekday schedule and occasional on-call/weekend availability.

Qualifications

• RN License is required.
• Valid driver’s license and reliable transportation.
• Minimum of 1-4 years of RN experience in hospice or home health.
• On-call rotation required.

Responsibilities

• Provide initial and comprehensive assessments; develop personalized care plans.
• Implement and revise care plans; ensure high quality care and services.
• Communicate with patients, families, and health teams to coordinate care.
• Supervise and instruct direct care staff and non-professional hospice aides.
• Participate in on-call rotation; provide 24/7 coverage.
