Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for hospice services with Community Loving Care Hospice, part of the LHC Group family within Optum. The role covers full nursing responsibility for Plan of Care, collaboration with professionals, and delivering comprehensive care across physical, psychosocial, emotional, and spiritual aspects.

Qualifications

• Active RN license in the practicing state.
• Valid driver's license and access to a vehicle or public transit.
• 1+ year of clinical experience.
• CPR certification or ability to obtain within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining hospice eligibility within 48 hours of referral.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and responses to care.
• Collaborates with patient/family, attending physician, and IDG members daily.
• Instructs and supervises patient/family in self-care strategies when appropriate.
• Maintains accurate clinical notes on patient condition.
• Function in diverse home environments irrespective of background.
