Trinity Health At Home is seeking a Hospice Registered Nurse Case Manager in California. The role includes performing comprehensive assessments, developing personalized care plans, and coordinating with an interdisciplinary team to deliver compassionate at-home care.

A $10,000 sign-on bonus is offered and benefits start Day One. The position requires an active RN license (BSN preferred) and at least 1 year of nursing experience, with home health experience preferred.

Your goal is to maximize the comfort, dignity, and well-being of patients and their families in alignment with the interdisciplinary plan of care and all regulatory standards. In the role of Hospice Admission RN, you will also be responsible for initiating the hospice plan of care and ensuring clear, timely communication in accordance with established guidelines.

What You’ll Do

What You Bring

• Current RN license (BSN preferred)
• Minimum 1 year of nursing experience (required in California)
• Home healthcare experience preferred
• Valid driver’s license and reliable transportation
• Strong computer skills and a commitment to ethical, mission-driven care

Physical & Mental Requirements

• Frequently lift, stand, and walk (35% of time)
• Occasionally bend, stoop, twist and assist with patient positioning (10%)
