St. Croix Hospice in Illinois is seeking a Registered Nurse Case Manager to provide 1:1 holistic support to patients and families in home settings.

You will partner with an interdisciplinary team to develop personalized care plans and guide end-of-life conversations with professionalism and empathy. The role emphasizes compassionate patient connections, autonomy in practice, and coordinating care across shifts.

Qualifications

• RN licensure in good standing.
• 1+ years acute care or hospice/home health experience.
• Ability to perform comprehensive assessments and educate patients.

Responsibilities

• Perform initial and ongoing assessments to determine hospice needs.
• Develop and update individualized care plans with patient and family.
• Administer medications and treatments as prescribed.
• Rotate on-call duties and ensure seamless care transitions.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.

Education

RN licensure (associates/bachelors/masters in nursing)
