Care Initiatives Hospice is seeking a compassionate Registered Nurse (RN) Case Manager to deliver high-quality, patient-centered care in patients' homes or other settings. You will develop and oversee plans of care under physician direction, perform comprehensive assessments, and conduct ongoing interdisciplinary care visits.

The role requires an Iowa RN license and CPR certification, hospice experience, and willingness to travel within the region. CHPN within 24 months is encouraged.

Qualifications

• Active Iowa RN license required.
• CPR certification required.
• Hospice or palliative care experience preferred.
• Two years of verifiable RN/LPN experience preferred.
• Valid driver's license and ability to travel within the region.
• Home internet access.
• Ability to obtain CHPN within 24 months of employment.

Responsabilités

• Provide patient-centered care at the patient's place of residence.
• Develop and oversee plan of care under physician direction.
• Perform initial and comprehensive assessments.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care with clear, concise clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated.
• Develop and update plan of care to meet patient-specific needs.
