Job summary

Care Initiatives Hospice is seeking an RN Case Manager to deliver compassionate, patient- and family-centered hospice care across patients’ homes, long-term care communities, assisted living residences, or hospitals. You will assess, plan, and coordinate interdisciplinary care with a focus on comfort, dignity, and respect during end-of-life care.

Ideal candidates hold an Iowa RN license and CPR certification, with hospice experience and the ability to travel within the region.

Qualifications

• Active Iowa RN license is required.
• CPR certification is required.
• Hospice or palliative care experience is preferred.
• Two years of verifiable nursing experience (RN/LPN) preferred.
• Must have a valid driver’s license and be able to travel between locations.

Responsibilities

• Perform initial and comprehensive patient assessments in patients' homes or facilities.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Conduct continuous visits and interdisciplinary evaluation of patients.
• Provide accurate clinical documentation and care coordination.
• Deliver counseling and education to patients, caregivers, and families as indicated in the plan of care.
• Update the care plan as patient needs evolve.
