Job summary

Interim HealthCare of West Michigan is seeking a Registered Nurse to join our Hospice division in Michigan. This role offers flexibility, patient advocacy, and a supportive team where management backs you and nurses are valued.

Responsibilities include ongoing patient assessments, developing care plans with the IDG and family, pain management, ensuring supplies and documentation, and regular IDG participation.

Qualifications

• Active, unrestricted Michigan RN license.
• Minimum 1 year of RN experience.
• Home health experience preferred.
• Current CPR certification.
• Reliable transportation with valid driver's license and insurance.

Responsibilities

• Assess the patient’s physical and psycho-social status.
• Develop and revise the plan of care with the IDG, patient/family and attending physician.
• Provide education and interventions for pain and symptom management.
• Ensure availability of equipment, supplies, medications and services.
• Maintain accurate and timely documentation of services provided.
• Attend IDG meetings regularly.
