Job summary

Interim HealthCare Home Health and Hospice seeks a Hospice RN Case Manager to deliver compassionate, autonomous nursing care in patients' homes. As a registered nurse with responsibility for care plans and coordination, you will partner with physicians and families to implement effective, patient-centered care.

Qualifications

• RN license active and unrestricted in Michigan.
• Minimum 1 year of clinical RN experience, preferably in Hospice.
• Current CPR certification.
• Reliable transportation with valid driver's license and auto insurance.

Responsibilities

• Provide home-based skilled nursing care to patients with terminal illness.
• Collaborate with physicians and oversee CNAs delivering patient care.
• Conduct patient assessments, document changes, and report to the physician.
• Educate patients on plan of care, medications, diet and exercise.
• Coach family members on care plan, medications, and home safety.
• Coordinate discharge from services and ensure goals are met.

Hospice NurseResponsibilities:

• Provide home-based care to patients with terminal illness
• Work with a team of physicians and oversee CNAs providing patient care
• Examine patients, do assessments, document and report changes to their physician
• Educate patients on their plan of care, prescribed medication, diet and exercise
• Coach family members on the patient’s plan of care, medication and home safety
• Ensure goals are met and coordinate discharge from services

A few must-haves for Hospice Registered Nurses:

• Registered Nurse (RN) with active, unrestricted license in the State of Michigan – BSN not required
• At least one (1) year of clinical experience as a Registered Nurse (preferably in Hospice)
• Current CPR Certification
• Reliable transportation with a valid state driver's license and current car insurance
