Interim HealthCare of West Michigan in Grand Rapids is seeking a Registered Nurse for our Hospice division. This weekend-only position begins Friday 4:30 PM and runs through Monday 8:00 AM on-call, with ongoing patient assessments and care planning.

You will work with the IDG to develop and adjust care plans, educate patients and families, and manage pain and symptoms while ensuring equipment and medications are available.

Qualificações

• Active, unrestricted RN license in Michigan.
• At least 1 year of RN experience.
• 1 year of management experience preferred.
• 1 year of home health experience preferred.
• Current CPR certification.
• Reliable transportation with a valid driver’s license and insurance.

Responsabilidades

• Perform ongoing assessments of the patient’s physical status and psycho-social status.
• Initiate and develop a plan of care with IDG, patient/family, and attending physician.
• Provide education and interventions for pain and symptom management and monitor response.
• Ensure equipment, supplies, drugs and other items are available.
• Make revisions to the IDG plan as needs change and document services timely.
• Attend IDG meetings regularly.
