The ideal candidate will bring clinical education, state licensure, and at least one year in home care, plus comfort with regulatory guidelines and mobile technology.

Qualificações

• Graduate of an approved school of clinical education and licensed in the state.
• At least 1 year experience in home care.
• Understanding of federal, state and local hospice laws and guidelines.
• Basic demonstrated technology skills, including operation of a mobile device.

Responsabilidades

• Represent the branch in external transitional care activities and professional contacts to promote Enhabit services.
• Develop patient-centered transition plans by integrating clinical guidelines and metrics.
• Serve as a public awareness representative and educate about provided services.

Formação académica

Graduate of an approved school of clinical education

Licensed in the state of practice

At least 1 year experience in home care

Knowledge of federal, state, and local hospice regulations

Basic technology skills (mobile devices)
